So occasionally, I have Overdrive if I fill the car up with premium, let it warm up and/or step on it while on the highway, but on some days, the O/D would never be engaged at all. So now I’m at a loss as to what’s going on with the ES300, but I’m pretty sure O/D should have been working until now.
Either that, or I need to change the fluid soon... I hate this fucking car.

tl;dr: It has intermittent O/D issues where it just doesn’t want to work most of the time.
Also, I just checked the fluid. Shit. It’s black.
I hate myself.

Do you always fill it up with at least 91?

Not always, but now that I think about it, I think the last two gas stations I've visited before the issue reared its ugly head were ARCO and Safeway because I got cheap... I fucked up, huh?

Big time. 1MZ-FEs can’t run on anything lower, as the knock sensor will send screwy signals to the ECU and all kinds of weird and expensive things can happen. Ask me how I know.

So how much am I looking at for that?

Drain the tank, fill it up with clean hi-test and a bottle of Lucas Deep Clean Fuel System Cleaner (must be the deep clean version). Disconnect the battery and let it sit for 48 hours to clear the memory cache. Start it up in the evening, and let it idle until warm. Then, when the sun is completely down (denser charge, minimizes legal risk), give it the Italian Tune-up of it’s life until the tank hits 1/4. Then, just fill it up with fresh hi-test and you should be good. DON’T CHANGE THE TRANS FLUID, THE ONLY THING KEEPING YOUR TRANSMISSION TOGETHER ARE THE PARTICLES OF FRICTION PAD IN THE FLUID.

So I just got the car back after 48 hours and after the above method it’s still doing the thing - no overdrive and it’s flashing P0330 on the reader.

Damn, I guess it was too far gone for even the Italian tune-up on premium to save. That would be the bank 2 knock sensor, about $150 in parts. You have to pull apart the manifold to get to it, and I’d trace all the wiring for the sensor just to make sure there aren’t any breaks or shorts. It’s all three of my least favourite things. An oddly expensive part, fiddly work, and a time-consuming process. Sorry to be the bearer of bad news.
